Yes. I use special tracking software that's very good at tracking clicks from a PDF. Some click-tracking solutions do not track clicks from a PDF, only from website links.

Yes. It will look exactly the same as the one I use, except your name and website link will replace my name and website link. Also, your affiliate links for the Co-Branded version of the Toolbox, for the Systeme marketing platform, and for the 10 recommended product links will replace mine.
No. You won't even have to open a word processing document to customize the Toolbox for your audience. You'll just sign up for the affiliate programs as instructed, then go to a page where you'll find a form to fill out. Don't spend too much time trying to get approved for affiliate programs in the Toolbox.
